Just a note, the R5RS seems to have it right.  The HTML output
becomes:

<div align=left>
  <u>library procedure:</u>
  &nbsp;&nbsp;
  <tt>(<a name="%_idx_540"></a>vector<i> obj <tt>...</tt></i>)</tt>
  <!-- ^ anchor is placed here -->
  &nbsp;
</div>

Though the (La)TeX source of R5RS doesn't seem to be available at
schemers.org.
